Medical History* – This document can be completed by the parent/guardian of the applicant.
Records Release Form – This form must be delivered to the applicant’s current school. We accept records via fax, mail, and email.
Student Recommendation Forms– Each applicant must have three recommendations from his current school. These forms should be filled out by the applicant’s math or English teacher, counselor, or a school/church faculty member who can deliver an accurate recommendation of the student.
For international students, we will also require transcripts (translated into English, past three years), a copy of passport biography and photo page, and Official TOEFL/SSAT or IELTS scores. Please note that before we issue an I-20 we also require a statement of financial support and bank statements showing the source of funding for the current school year.
